Are you attending the Hamilton Medical Group for a COVID-19 Vaccination and haven't previously completed one of our COVID-19 consent forms?

There are multiple ways you can do this prior to your appointment:

1. Via a link in your confirmation Email

2. Via a link in your SMS appointment reminder

3. Via the QR Code below (new!)

Consent forms must be completed prior to attending for your vaccination.

Fees

The Hamilton Medical Group is a private medical practice, not a bulk billing clinic. Discounts apply for fees that are paid on the day of consultation. Please stop at the accounts desk after every appointment.
